Services/VNA is pleased to offer a program called the Honeywell
HomMed® Home Monitoring
System which is available in Columbia, Montour, Luzerne and
Northumberland Counties. The Honeywell HomMed®
Monitoring System provides daily at-home monitoring of vital
signs for patients with various health conditions. The system
gives the users the best of both worlds, the comfort and independence
of staying at home and the security of daily monitoring and
follow-up medical attention if necessary.
How Does The Monitor
Work?
The Honeywell HomMed®
System is comprised of two main components: the home unit that
is placed in the patient’s home and the central station.
The home unit is a vital-sign monitoring and communication device
that measures weight, blood pressure, heart rate, temperature
and other tests. The data is automatically sent to the central
station which is located at Columbia Montour Home Health Services
where the nursing staff interprets the data.

Patients who are referred to Columbia
Montour Home Health Services/VNA and meet the selection criteria
will be monitored at no cost to the patient.
Patients not being followed by Columbia
Montour Home Health Services/VNA have the option to lease the
monitoring system at a nominal fee.
The Honeywell HomMed®
System can benefit anyone with a chronic illness or any other
