Columbia Montour Home Health Services/VNA
in collaboration with Central
Susquehanna Community Foundation is pleased to announce the
Nurse-Family Partnership which is available in Columbia, Montour,
Snyder, Lower Luzerne and Upper Northumberland Counties.
The nurse home visitation model is based on the work of Dr. David
Olds at the National Center for Children, Families and Communities
(NCCF). The three goals of the program are to:

Improve health during pregnancy
- Nurses help new mothers-to-be improve their diet and
nutrition, utilize proper obstetrical care, and avoid use
of tobacco or other harmful substances.

Promote healthy child development
- First-time parents learn how to provide responsible and
competent care, particularly in the areas of children’s
physical and emotional health and safety. Nurses help
parents build a strong support network of family and friends.

Promote a positive maternal life course
- Together, nurses and parents create a vision for the
future that includes a plan for future pregnancies,
steps toward continuing education, and securing employment.
